For physiotherapy, you will need a referral from your general practitioner, specialist, or hospital doctor. Therapies can be prescribed for 30 minutes, 45 minutes, or 60 minutes. On this form, in addition to the diagnosis and type of therapy, the number and duration of the therapy should be noted (e.g. individual physiotherapy 6 x 45 min.), as well as any required home visit.
